Two different problems. Two different routes.
Understand the eight life-changing events, the distinction between a revised determination and reconsideration, and the evidence to organize before contacting SSA.
A spouse needs their own request
This page does not decide your appeal
New initial determination versus formal reconsideration
| Question | New initial determination | Formal reconsideration |
|---|---|---|
| What is it? | A request for SSA to redetermine IRMAA using more recent income or a changed filing situation after a qualifying event. | A review request when you disagree with an SSA determination, including an IRMAA determination where the reconsideration route applies. |
| Typical trigger | One of SSA's life-changing events, such as work stoppage, work reduction, death of a spouse, marriage, divorce, or a qualifying pension or property loss. | SSA used information you believe is wrong, or you disagree with the determination after reviewing the notice and the rules. |
| Official form | SSA-44 is the form SSA provides for reporting a qualifying life-changing event. | SSA-561 is the form used to request reconsideration. |
| What it is not | Not a guarantee of a lower tier, and not the same as a formal appeal. SSA makes the decision. | Not an SSA-44 life-changing-event request, even if the underlying concern is income-related. |
The eight events the navigator must recognize
| Event | What to investigate | Evidence categories |
|---|---|---|
| Death of spouse | The death changes relevant income, filing status, or household circumstances. | Death certificate or SSA-recognized death record; updated income evidence. |
| Marriage | Marriage changes the filing status and the MAGI used for the determination. | Marriage certificate or acceptable equivalent; relevant tax information. |
| Divorce or annulment | A legal termination changes household income or filing status. | Divorce decree or annulment order; updated income evidence. |
| Work stoppage | The beneficiary or spouse stops working. | Employer letter, separation evidence, or an acceptable signed statement. |
| Work reduction | The beneficiary or spouse reduces work hours, such as moving from full-time to part-time work, with a qualifying reduction in MAGI. A pay cut alone is not the work-hours event described here. | Employer verification, reduced-work documentation, or an acceptable sworn statement. |
| Loss of income-producing property | A qualifying involuntary loss of property that produced income. | Insurance or disaster records, theft/fraud records, and income evidence. |
| Loss or reduction of pension income | A pension plan terminates, ceases, or is reorganized in a qualifying way. | Pension administrator statement and income records. |
| Employer settlement payment | A settlement is associated with an employer closure, bankruptcy, or reorganization. | Settlement agreement, employer explanation, and payment records. |

Income that increased temporarily is not automatically a qualifying event
SSA lists several circumstances that ordinarily do not qualify as life-changing events merely because they temporarily increased income:
- Roth IRA conversions
- One-time capital gains from selling property
- Lottery winnings
- Casino winnings
- Cashing bonds
- Ordinary investment-income losses
- Higher household or medical expenses
A home sale with a taxable gain does not, by itself, become an SSA-44 event because income later falls. The navigator must still screen for independent qualifying circumstances, tax-data errors, and any available formal appeal rights.
Not every problem is an SSA-44 question
Amended tax return
SSA's process may consider qualifying amended tax documentation. Gather the amended return and proof of IRS acceptance.
Corrected IRS information
If the IRS data SSA used is wrong, the route is a corrected-data request, not a life-changing event.
Older tax year used
SSA may use three-year-old information when the normal return is unavailable. A newer available return may support a different determination.
MFS and lived apart
An attestation of living apart for the entire tax year can support the individual schedule, but only for the spouse who attests.
What to gather before contacting SSA
- The IRMAA notice and the premium year it covers.
- The tax year and filing status SSA used.
- The event type and the month/year it occurred.
- Revised AGI and tax-exempt interest, if known.
- Evidence categories that fit the event, not a generic document list.
- The deadline language printed on the notice, if you are considering reconsideration.
Do not enter a full Social Security number here. If you complete an official form, enter identifiers privately in the official document.
